To determine the status of this request, if not notified within 2 weeks, contact the Interlibrary Loan office at 973-761-9441. Books not picked up by the date due will automatically be returned to the lending library and cannot be requested again.

Comments or Notes:

Additional forms may be submitted without re-typing identical information (name, etc.) by selecting and deleting the text in each field to be replaced with different information. "Refresh Screen" clears all fields.
