Connections at Home New Jersey is a non-profit organization associated with Vanguard Medical Group, whose mission is to use technology to help decrease social isolation among the elderly, and assist patients and their families strengthen ties and stay connected. Lifeline, an FCC program, provides low income subscribers a discount on monthly telephone service purchased from participating providers in the marketplace. WIC is a preventive public health nutrition program that provides nutrition and breastfeeding education, nutritious foods, and improved access to regular health care and social services to low and moderate-income pregnant, postpartum and breastfeeding women and young children with, or at risk of developing nutrition-related health problems.

The Senior Farmers’ Market Nutrition Program (SFMNP) provides low-income seniors with access to locally grown fruits, vegetables, honey and herbs.

For programs that prevent homelessness and help people move out of temporary shelters and into stable, permanent housing. The Housing Assistance office administers housing assistance programs such as the Homelessness Prevention Program (HPP), the Housing Choice Voucher Section 8 Program (Section 8) and the State Rental Assistance Program (SRAP).

NJ Find A Ride is an online directory of public and accessible transportation options for those who do not own a car. The website offers a place to look at how to get from one location to another in a local community through a searchable database. The search results will match specific transportation needs that you describe and state who is eligible, how much it costs and how to arrange for a ride. Local and community transportation providers can add and update their services through links at this website.
